Supplemental Security Income - SSI

Applying for a Child

What you should know before you apply for SSI Disability benefits for a child? PDF

To apply for a child, you can start the application online HERE or by calling the Social Security Administration at 1-800-772-1213, or visiting their local office. 

If applying online, you must contact the Social Security Administration by phone or in person to complete the application process by calling 1-800-772-1213.

Once you apply, you will be contacted for an intake appointment. You will need to fill out and bring with you the following worksheets:
